This bill requires health insurance carriers which offer dental plans to use a universal dentist application for participation form, and a form for renewal of credentialing, to be adopted by regulation of the Commissioner of Banking and Insurance for the purpose of credentialing dentists who seek to participate in the carrier's provider network and for the purpose of credentialing dentists who are employed by hospitals and other health care facilities which seek to participate in the carrier's network. The bill provides that in developing the universal application form and the renewal form, the Commissioner of Banking and Insurance must consult with the Commissioner of Human Services to ensure that the credentialing requirements for the Medicaid and NJ FamilyCare programs are adequately reflected on the forms. The bill directs the Commissioner of Banking and Insurance to adopt the forms, by regulation, within 180 days of the date of enactment of the bill. The bill provides that a dental plan must accept the universal application and renewal forms, within 180 days of their adoption by the Commissioner of Banking and Insurance, for the purpose of credentialing dentists who seek to participate in the carrier's provider network. The bill also provides, however, that a carrier may request additional information from an applicant that is not provided for on the universal application or renewal form, as applicable, if the requested information does not duplicate any information included in the form.
