This bill amends the Social Services Law and the Public Health Law to require an independent assessment and person-centered service plan for individuals receiving long-term care assistance through Medicaid. The bill mandates that the assessment and service plan be conducted by an independent assessor, rather than the managed long-term care plan, to ensure objectivity. The service plan must consider the individual's psychosocial, cognitive, and medical needs, as well as their functional level and support systems. The effectiveness of the service plan will be monitored and revised as necessary, at least annually. The bill also repeals a provision related to comprehensive assessments for managed long-term care plan enrollments.
