This bill establishes an intensive case management program within the Massachusetts home care program to support individuals who are clinically complex and require sustained, in-depth case management exceeding the level provided under the regular home care program. The bill defines "clinically complex" as a summarized status of an individual's clinical eligibility for receiving intensive case management. The intensive case management program aims to help transition individuals into receiving home care services or retain active enrollees who are experiencing participation challenges due to serious mental illness, complex family dynamics, or multiple medical and functional needs that put their ability to be appropriately served at risk. The Department of Elder Affairs is required to establish reimbursement rates for the intensive case management services that reasonably reflect the level of expertise and time commitment required to support qualifying individuals.
