This bill aims to improve discharge opportunities from acute care facilities. It requires that patients or their representatives be presented with a written review of post-acute care options at least 24 hours prior to discharge, with the highest priority given to returning the patient to their residence or community with appropriate services. In cases where this is not deemed possible, the acute care facility must provide a written explanation and certify it as true and correct. Additionally, the bill expands the requirements for discharge plans to include not only Medicare, but also MassHealth (the state's Medicaid program) and private commercial insurance licensed in Massachusetts.
