This bill aims to establish a community-based palliative care model under the Center for Medicare and Medicaid Innovation (CMI). The model will provide palliative care and care coordination to high-risk Medicare beneficiaries with serious illnesses or injuries, such as cancer, heart disease, or Alzheimer's. The model will be implemented for a 5-year period and will include a multidisciplinary palliative care team that works in coordination with primary and specialist care providers. The goal is to improve patient outcomes, reduce unnecessary emergency department visits and hospitalizations, and ensure equitable access to palliative care, including in rural and underserved areas.
