This bill aims to increase access to portable orders for life-sustaining treatment (POLST) by: 1. Establishing a statewide registry for POLST forms, where healthcare providers and individuals can submit forms to be digitally stored and accessed by authorized parties like personal representatives and emergency responders. The registry will provide individuals with a registration number and allow them to review and revoke their POLST forms. 3. Allowing healthcare providers to submit POLST forms to the registry on a patient's behalf, unless the patient opts out. The bill also updates provisions related to the existing statewide health care declarations registry to align with the new POLST registry. The bill will only take effect if specific funding is provided in the state's omnibus appropriations act.
