This bill mandates that healthcare professionals providing prenatal care must offer patients the opportunity to develop a personalized postpartum care plan, which includes contact information for their care team, scheduling details for postpartum visits, breastfeeding guidance, reproductive life planning and contraception advice, information on pregnancy complications and follow-ups, guidance on signs and symptoms of postpartum depression or anxiety, strategies for managing psychiatric issues, recommendations for managing postpartum physical issues like incontinence or pain during intercourse, and a treatment plan for ongoing health conditions. This plan should be discussed ideally during the first trimester, and if a patient declines or doesn't have a plan, they must be informed of the risks of inadequate postpartum care. Additionally, hospitals, ambulatory care facilities, and birthing centers providing maternity services must give patients information on postpartum care, pregnancy loss, and stillbirth, based on best practices, before they are discharged. The bill defines "care team" as a group of healthcare professionals, the patient, and their support network, and "medical home" as a primary care provider or facility that maintains a patient's medical information and provides access to care.
