The form must also inquire whether the applicant has ever been arrested, charged, indicted, or convicted under N.J.S.2C:14-2 (Sexual Assault or Aggravated Sexual Assault) and N.J.S.2C:14-3 (Criminal Sexual Contact or Aggravated Criminal Sexual Contact).
