This bill establishes a three-year pilot program designed to improve healthcare coordination for veterans who are enrolled in both Medicare and the Department of Veterans Affairs (VA) healthcare system, known as "covered veterans". The program will be administered through the VA's Center for Innovation for Care and Payment and implemented in 3-5 Veterans Integrated Service Networks that represent diverse geographic areas, including rural and medically underserved regions. Each participating veteran will be assigned a dedicated case manager responsible for developing an individualized needs assessment and care coordination plan, helping veterans navigate both VA and Medicare healthcare systems. The pilot program aims to enhance healthcare access, improve care outcomes, lower costs, eliminate service gaps, and better coordinate patient information across providers. The VA will be required to track detailed metrics about the program, including patient and provider satisfaction, care quality, access standards, and cost effectiveness. The Secretary of Veterans Affairs must submit quarterly reports during the first two years and annual reports thereafter, with a final report recommending whether the program should be extended or made permanent. The VA is encouraged to collaborate with private sector healthcare entities to design and implement the program, drawing on existing value-based care models to maximize effectiveness.
