The coverage is limited to three treatment cycles per lifetime and includes specific medical services such as office visits, hysterosalpingograms (a type of X-ray to examine the fallopian tubes), pelvic ultrasounds, and blood testing. The bill defines infertility based on the inability to establish a clinical pregnancy after 12 months of regular, unprotected sexual intercourse for individuals 21-34 years old, and after six months for individuals 35-44 years old.
