Specifically, when a patient with disabilities is being discharged from acute, chronic, or long-term care, the hospital must communicate the discharge plan to a registered or licensed practical nurse at the patient's residence or residential facility. For clients with intellectual or developmental disabilities, the receiving nurse must carefully review the discharge plan, with special attention to maintaining life-supporting technology and equipment, and must sign the plan to confirm full understanding. The nurse is also required to advise residential facility personnel, family members, and personal care attendants about the discharge requirements to ensure the client's continuous protection and safety. Additionally, the Department of Public Health must develop implementation guidelines, conduct public awareness campaigns, and publish informational materials on the bill's provisions. The bill mandates that the detailed discharge plan include specific information such as follow-up medical appointments, descriptions of required medical technology, maintenance instructions, and other details the department may require. The legislation will take effect immediately upon passage.
