The selected provider networks must be nonprofit organizations with a proven track record of supporting healthcare delivery in rural communities and the ability to collect and evaluate data. The bill amends the Social Security Act to mandate CMMI to develop and implement this model, which aims to connect patients with specialty care providers through remote technologies while coordinating with their primary care providers. Any funding for this initiative will be subject to existing funding requirements for public health service programs.
