This bill requires a mandatory residential transition conference when a person with developmental disabilities moves between residential programs operated by different "covered operators" (organizations certified to provide residential services for people with developmental disabilities). The conference must include specific attendees: the current and future residential program operators, the person with developmental disabilities (unless their health or safety would be compromised), their family (unless the individual objects), their guardian or legal representative, and their care manager. The primary purpose of the conference is to review the individual's "life plan" - a comprehensive document outlining the person's goals, needs, preferences, and support strategies - and allow attendees to ask questions or provide additional information. After the conference, both covered operators must complete and sign a standardized form summarizing the discussion, which must be submitted to the office for people with developmental disabilities. Copies of this form will be provided to the individual, their legal representative, and both residential program operators.
