The program will involve a technology vendor providing FDA-authorized devices to collect health data like blood pressure and glucose levels, which will be monitored by a remote care team consisting of nurses, dietitians, and diabetes educators who will also offer health coaching and coordinate with healthcare providers. An "escalation pathway" will be in place to address concerning measurements, and the program will operate for two years, aiming to enroll up to 500 participants, including those in areas with limited maternity care access. The department will fund the program and, within two years of its start, must submit a report to state officials evaluating its impact on maternal, fetal, and neonatal health outcomes, potential Medicaid savings, and recommending whether to expand the program statewide.
