This bill requires the Division of Aging Services (the division) in the Department of Human Services to establish one application form that a person aged 65 or older or between the ages of 18 and 64 and receiving Social Security Disability, and who is eligible to receive services from a federal and State-funded program administered by the division can use in applying for such services. The form is to include all information necessary to determine eligibility for all the programs administered by the division.
