This bill requires a "residential transition conference" to be held whenever a person with developmental disabilities moves from one residential program run by a "covered operator" to another, or vice versa. A covered operator is defined as any entity holding an operating certificate for residential services, an organization needing commissioner approval for residential services, or any state-funded or administered program providing residential services. The conference must include the current and future covered operators, the individual with developmental disabilities (unless medically or safety contraindicated), their family (unless the individual objects), their guardian or legal representative, and their care manager. The primary purpose of the conference is for the current operator to review the individual's "individualized service plan" or "life plan" – a document outlining their goals, needs, and services – with the future operator and allow attendees to ask questions or provide input. Following the conference, both operators must complete and sign a summary form, which is then filed with the office for people with developmental disabilities and copies are provided to the individual, their representative, and both operators.
