This resolution requests the Department of Health to establish a working group focused on "complex needs patients," defined as individuals with multiple significant but not severe chronic conditions, including substance use disorders, mental health disorders, or chronic physical illnesses. The working group, to be chaired by the Director of Health or their designee and include various stakeholders, will examine the needs, services, and funding required for these patients, who often experience fragmented care leading to poor outcomes and high costs. Specifically, the group is tasked with considering integrated residential and therapeutic living facilities, comprehensive substance use and mental health treatments, primary and specialty medical care coordination, nursing services, care coordination, peer support, recovery-oriented programming, and assistance with social determinants of health like housing and employment. The Department of Health is then required to report its findings and recommendations, including any proposed legislation, to the Legislature by early 2027.
