To support the request, applicants must submit required forms and provide acceptable documentation, which can include a medical service provider's statement, a certified court order, or a sworn statement affirming the request is made in good faith and not intended to defraud. Once the birth record is replaced, the original record will be classified as confidential and can only be disclosed through a court order.
